Inpatient & Swingbed
Hours of Service: 7 Days, 24 Hours
Community Hospital Association provides inpatient care for acute care, skilled care,
and intermediate care patients. Each level of care requires admission orders from
a member of the Active Medical Staff. All admissions are reviewed by our Utilization
Manager and the patient s insurance carrier. All Medicare and Medicaid admissions
are reviewed in accordance to Federal Regulations.
A patient may also be admitted as an observation patient if ordered by the family
nurse practitioner or their physician. This service is to provide nursing observation,
administration of needed medications, or follow up testing to help determine a diagnosis.
The patient must be discharged, transferred, or admitted to acute care within 24
hours.
Swingbed admissions should be prearranged with our Skilled Care Coordinator
or Utilization Manager. A patient plan of care and discharge summary will be required
prior to admission. A member of our Active Medical Staff must accept the transfer
in advance. The Swing Bed Program at Community Hospital -Fairfax benefits patients who need
rehabilitation services or extended medical care. Patients who have had three
days of acute care and will be under the care of an active member of our medical
staff may qualify to receive short-term skilled care at CH -F. Admission criteria
for the swing bed program include physical therapy, occupational therapy, speech
therapy, nutritional services and IV therapy. Other services provided are
skilled nursing, pharmacy and social services. This program enables patients
to receive the care they need close to home, with their primary physician and familiar
staff.
